We don't.



Transsexual people have no wish to change gender; we're quite happy with the gender we were born with. It's our sex we have a problem with.



Gender is not the same thing as sex, although the two are often used interchangeably.



As to how it happens....it's a long, complicated process, involving social changes, hormone therapy, and surgery.How do transexuals . change there gender?

I don't really know the specifics..but I would imagine that it is as much an emotion 'surgery' as a medical 'surgery'...wait for someone on here who knows more about it than I do...they'll give you really good answers.

most FTM transsexuals only opt for 'top' surgery to remove their breasts and give them a masculine chest.

'bottom' surgery is much more difficult for girl to guy although still possible, however the results are far from ideal, often small and sexually useless .. or at least that's what I've heard from people in the past... do some googling for check out the wikipedia if you're really curious.

Also FTM will take testosterone, which has a number of effects, but basically they go through puberty.
The last I heard, gender is in reference to what is in between the ears, the inner person. I think you are refering to changing physical appearance of sex: male secondary characteristics to female secondary characteristics and visa versa. For a physical woman to become a man, there is hormones and surgeries involved. For a physical male to become a woman, there is a long list: electrolysis, voice therapy, learning how to apply makeup, walking, dressing (if they don't know already), and surgeries as well.
Their physical sex gets changes to match their gender identity. This basically involves socially joining the team they identify with and, to whatever extent they can and/or want, change their body to match.

The point about a transsexual as opposed to a transvestite is that while they have the body of one sex they have the brain of the opposite sex. Male to female transsexuals are more common as we all start of as females but the male foetus is turned into a male by supplies of testosterone. so if the testosterone fails for some reason they stay female or partly female. The female to male transsexual is far less common as and excess of testosterone is required which is not so likely to happen. So the persons brain tells them they are a different sex to their body. They can't alter their brain so they try to alter their body. How far they go with this is up to the individual . Many are content to use hormone treatment to achieve some different body shape but some want full surgery which is of course a lot easier for male to female than it is for female to male
